Specimen Requirements
Submit only 1 of the following specimens:
Plasma
Draw blood in a green-top (lithium heparin) tube. (Hemolyzed specimen is not acceptable.) Spin down and send 2 mL of lithium heparin plasma. Plasma for a peak level should be drawn 30 minutes after completion of a 30 minute infusion or within 15 minutes after completion of a 60 minute infusion of an intravenous dose or 60 minutes after an intramuscular or oral dose of tobramycin.
Note: 1. Indicate plasma.
2. Label specimen appropriately (plasma).
Serum
Draw blood in a gold-top serum gel tube. (Hemolyzed specimen is not acceptable.) Spin down and send 2 mL of serum. Serum for a peak level should be drawn 30 minutes after completion of a 30 minute infusion or within 15 minutes after completion of a 60 minute infusion of an intravenous dose or 60 minutes after an intramuscular or oral dose of tobramycin.
Note: 1. Indicate serum.
2. Label specimen appropriately (serum).
Reference Values
Therapeutic range: 5.0-10.0 mcg/mL
Critical value (automatic call-back): >12.0 mcg/mL
Note: Variable time to peak concentration may be delayed in patients with abnormal renal function or large volume of distribution. Inactivation of aminoglycosides can be caused by high concentration of beta-lactam (eg, penicillin, cephalosporin).
